Mr. Perry Ray Chavis age 47 was born on June 2, 1973 to the late Joseph Chavis and the late Rose Merl Brooks in Scotland County, North Carolina. He departed this life to eternal life on Sunday September 20, 2020 at his home. Along with his parents he was preceded in death by a son Perry R. Chavis Jr., a brother Corey Daniels, maternal grandparents William and Virginia Brooks, paternal grandparents Raymond and Inez Chavis.

He leaves to cherish his memories, his wife, Shawana L. Chavis of the home, two sons, Logan P. Chavis of Alabama and Perry Ray Chavis Jr of the home. two stepdaughters, Isabella Coe of Shannon, North Carolina and Leiloni Cox of Alabama, one stepson, Sabastian Locklear of Shannon, North Carolina; fours sisters: Ginger Chavis, Renee Chavis, Shawnie Henderson, and Krystal Locklear all of Red Springs, North Carolina. two brothers, Joseph Chavis and Dusty Chavis both of Red Springs, North Carolina. stepmother, Freida Daniel and step grandmother, Pastor Audry Hall Jacobs. two aunts, Kathy Locklear (William), Delphen Suggs, three uncles: William Brooks (Pearlie), Eddie Brooks and Harold Brooks. twelve nieces, two great nephews, and two great nieces and a host of other family and friends.
